    Here is the word from Jim Carroll, the MyHIPAA Guide risk assessment expert:

    Patient Privacy Notices should be signed by patients at least once a year, but it is not necessary to ask patients to sign every time unless there are changes to the Privacy Notice. If there are changes, then all patients will need to re-sign at the time of service.
